Job Summary

The Nurse Practitioner-Dentist (NPD) Model for Primary Care is an innovative, interprofessional collaborative practice model that integrates primary care and oral health services at the Harvard Dental Center (HDC) Teaching Practices.

This position is a 10 hour/week LHT Hire appointment at Harvard School of Dental Medicine with responsibilities for the provision of primary care services at the HDC Teaching Practices. The Certified Nurse Practitioner (CNP) is Board-certified in adult-gerontological primary care and authorized to practice as an Advanced Practice Registered Nurse in Massachusetts. The CNP will collaborate with dental providers and other interprofessional team members in providing an integrated approach to patient care.

This is a 1 Year term appointment subject to annual renewal based on funding and performance.

Position Description

  • Collaborate with Program team members on the development, implementation, and evaluation of dental office processes and systems for medical-dental integration, including medical coding, billing and claims management.
  • Provide direct primary care services to adults and older adults within the HDC Teaching Practices, incorporating health promotion, disease prevention, and chronic disease management.
  • Conduct patient screening and risk assessment.
  • Identify patients without a primary care provider and/or last annual primary care visit greater than one year.
  • Assess patient health status, including Annual Wellness Visit in accordance with Medicare and other health insurance requirements.
  • Diagnose patient health status.
    • Identify patient-specific health conditions and medical treatments that impact oral health.
    • Identify patient-specific oral conditions and diseases that impact overall health and wellness
  • Plan and implement therapeutic interventions to optimize patient health and chronic disease management.
  • Collaborate with patient, family, and caregivers in the development of appropriate interventions.
  • Order tests and treatments.
  • Prescribe medications.
  • Make referrals to community-based primary care providers, specialists, sub-specialists, and community services.
  • Use electronic health record to promote effective communication, and interdisciplinary plans of care and treatment.
  • Provide patient, family and caregiver education. Integrate oral health information with diet and lifestyle behavior counseling. Educate individuals, families, and caregivers about:
  • Strategies to promote healthy aging and living well with chronic conditions;
  • Relationship between oral health and systemic health; and
  • Appropriate preventive measures to mitigate oral diseases and chronic conditions
  • Promote a patient- and population-centered, collaborative approach to optimize health outcomes and chronic disease management for adults and older adults.
  • Establish collaborative, interdisciplinary relationships to ensure effective care coordination and transitions of care for individual patients and populations served at the HDC Teaching Practices
  • Exchange meaningful information among health care providers within the HDC Teaching Practices and local health care delivery system.
  • Apply principles of interprofessional collaborative practice to ensure high-quality delivery of patient- and population-centered primary care and oral health care services.
  • Facilitate patient navigation in the health care delivery system through collaboration, communication, and appropriate referrals.
  • Attend and actively participate in regularly scheduled Program-related team meetings.
  • Collaborate with interdisciplinary team in developing implementation toolkit resources to promote medical-dental integration in dental practices.
  • Perform other tasks essential to effective Program implementation and evaluation

Basic Qualifications

  • Master’s degree in nursing, required. Nurse Practitioner certification in adult-gerontology, required. Doctor of Nursing Practice degree, preferred. Current or eligible nursing license to practice as an advanced practice registered nurse (APRN) in Massachusetts, required.
  • Two to five (2 - 5) years’ clinical experience as an APRN required. Experience providing comprehensive, primary care to adults and older adults with complex, multiple chronic conditions, preferred.
  • Ability to effectively coordinate comprehensive health care services across interdisciplinary sectors.
  • Proficient use of information systems and technology to support and improve patient care and healthcare systems.
  • Strong written and verbal communication skills; excellent teamwork and collaboration skills.

Working Conditions

Work Is performed in and office/clinical setting. The primary work location for this position is Boston, MA and in order to be on Harvard payroll, you must reside in one of the following states – MA, NH, RI, ME, CT, VT, NY, or MD. The Harvard School of Dental Medicine will support flexible work schedules subject to individual departments’ business needs.
